RN or LPN Nurse Care Manager - Split Position between N. Portland and Beaverton Clinics

Description:

Pacific Medical Group is a local, growing practice with five clinic locations in the Portland metro and surrounding area.  We are looking for a full-time Nurse Care Manager, a split position between our N. Portland and Beaverton clinics.

The primary responsibility of the Care Manager is to work collaboratively with Providers and staff to identify and support patients with chronic conditions and/or complex needs as defined by the practice. The principal objectives of the role are to facilitate communication, coordinate services, address barriers, and promote effective patient education and self-management. The development of care management processes, and support for the clinical teams in implementing these processes, are also an essential function of this role.

Essential Functions and Competencies:

  • Actively manage assigned panel of high acuity chronic patients

  • Collaborate with providers and staff in identifying patients for care management

  • Anticipate the needs of this patient panel, seeing that necessary documentation and pre-visit planning is completed before patient visit

  • Assess readiness to change and family resources for support

  • Coordinate development of an individualized treatment care plan

  • Promote patient self-management and empower patient/families to achieve maximum levels of wellness and independence

  • Educate the patient about self-management tasks to enable greater control of their health status

  • Provide follow-up with patients as indicated to ensure compliance with recommendations - medications, lab/x-ray, specialist visits, dietitians, etc.

  • Collaborate with the patient, provider and other team members in assessing the patient's progress toward individual health care goals

  • Assess barriers when treatment goals have not been met or failed important appointments

  • Coordination of care with other facilities and outside providers

  • Collaborate with payer Case Managers for additional services when appropriate

  • Maintain a list of community resources available to patients

  • Coordinate consistent documentation of patient self-management measures and reporting of progress towards treatment goals

  • Serve as a resource on care management to clinical teams

  • Facilitate pre-visit planning workflow

  • Develop a workflow to ensure Management of Clinic Operations

Qualifications:

  • Graduate of an accredited School of Nursing

  • Current Oregon Registered Nursing License or Oregon Licensed Practical Nurse

  • 2+ years of experience as a licensed RN or LPN

  • Relevant care management training may be substituted for experience requirement

  • Experience in a Primary Care Clinic setting preferred

  • Proficiency with windows-based EMR software as well as Word, Excel, Outlook & Access

  • Excellent written/verbal communication skills

  • Stable work history, with verifiable employment references/credentials.
