Simon Chan, DO joined the North Portland clinic in 1994. He received his medical degree from the Kirksville College of Osteopathic Medicine in 1982 and completed Family Practice residency training at Naval Hospital, Camp Pendleton. He served as Medical Officer as well as Director of Family Practice residency training during the ten years he served in the US Navy.

Dr. Chan enjoys seeing a variety of patients and treating all medical issues encountered in family medicine. He also practices osteopathic manipulation and acupuncture.

He and his wife have three children. He loves to read, travel, play basketball, volleyball and tennis. Dr. Chan still serves as a captain in the Naval Reserve one weekend a month.

Leslie Cody, FNP joined our North Portland clinic in December 2006. She completed her Masters in Nursing in the Family Nurse Practitioner program at Washington State University in May 2006 and received Magnum cum Laude. She became Board Certified in Family Practice in December 2006 because she enjoys treating the whole family and she likes the wide variety of patients and problems.

Leslie enjoys working in the garden, hiking, throwing pottery (on the wheel) and running marathons.

Douglas L. Eubanks, DO joined the North Portland clinic December 2006. He received his medical degree from Chicago College of Osteopathic Medicine, Midwestern University in June 1978. Dr. Doug (as he likes to be called) became Board Certified in Family Practice January 1995. He chose Family Practice because he wanted to be an old fashion family doctor seeing to all the needs of his patients from birth to the elderly. Dr. Doug enjoys cycling, swimming, running and singing.

Clare Green, DO joined the North Portland clinic in June 2007. She received her medical degree from the University of New England College of Osteopathic Medicine in June 1997. She completed her residency at Michigan State University in Muskegon, MI in 2000. She completed a fellowship in Neuro-musculoskeletal medicine in East Lansing, MI in 2001. Dr. Green became board certified in Family Practice in 2000 and Neuro-musculoskeletal medicine in 2005.

Dr. Green chose Family Practice because she enjoys working with patients of all ages. She enjoys skiing, rollerblading, and hiking. Dr. Green likes to spend her free time traveling and spending time with family and friends.

Eugene Stevens, MD joined our North Portland clinic in 2000. He received his medical degree from the University of Nebraska in 1969 and completed a General Practice Internship at St. Benedict’s Hospital in Ogden, Utah in 1970. He was a flight surgeon for the United States Army and also has experience as an Emergency Room and Occupational Medicine physician. Dr. Stevens is Board Certified in Family Practice. He enjoys skiing and scuba diving.
