Licensed Social Service Provider Full-Time - Tigard

Pacific Medical Group is a local, growing practice with five clinic locations in the Portland metro and surrounding area.  We are looking for a full-time Social Service Provider to work at our Tigard clinic.

The primary responsibility of a person in this position is to deliver social work services to at-risk patients and their families.  The Social Service Provider is assigned to a particular office and provides their patients with direct casework services, referral to community resources, initial psychosocial evaluations and crisis intervention.  Duties are to be performed in a courteous and professional manner that consistently demonstrates and promotes the values of Pacific Medical Group


  • Medical Social Worker (MSW), Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) preferred.  Candidates completing licensure within 12 months will be considered.

  • Bachelor’s or Master’s Degree from a school of social work accredited by the Council on Social Work Education

  • 2+ years recent clinical social work experience in an outpatient health care setting, preferably in the Portland Metro area

  • Knowledge of medical office processes – primary care clinic setting preferred

  • Command of medical terminology and common medical abbreviations

  • Familiarity with OSHA and HIPAA regulations

  • Competency and proficiency with windows-based EMR software (Intergy preferred), as well as Word, Excel and Outlook

  • Proficient English language skills (speaking, reading, writing, understanding)

  • Excellent written/verbal communication skills (grammar, spelling, sentence structure)

  • Impeccable customer service skills

  • Stable work history, with verifiable employment references


Essential Functions and Competencies

  • Establish and maintain a relationship of mutual respect, acceptance, and trust

  • Gather and interpret social, personal environmental, and health information

  • Establish achievable treatment goals with the client

  • Educate and support patients to facilitate cognitive, affective and behavioral changes consistent with treatment goals

  • Evaluate the effectiveness of treatment services provided to the patient

  • Identify appropriate resources and assessment instruments, as needed

  • Collaborate effectively with other social work or allied professionals

  • Collaborate effectively with providers and staff to develop treatment plans

  • Perform comprehensive initial psychosocial evaluations

  • Evaluate and treat problems within scope of practice
